* BITBPlus Discount Plan includes two adults (older than 18). For each additional family member, the annual membership is $200/yr (children under 18 are $150), for up to a total of four family members.

** Only one free whitening per patient per lifetime, AFTER completion of accepted dental treatment.

*** Additional Dental Services: We offer all BITB members a 12-15% discount on our regular fee schedule for any additional dental services. Noted veneers are excluded from the discount. A copy of the fee schedule can be provided to the patient upon their request. Noted that if this discount plan is purchased as a supplement to your insurance, your insurance’s fee schedule will be used for the additional dental services.

Disclaimer: This is not a dental insurance. This discount plan’s fee schedule can change upon annual renewal period & can be obtained upon patient’s request. Renewal is automatic every 12 months, unless patient notification of cancellation of plan 30-days prior to renewal date. The plan is non-refundable and cannot be combined with any other discounts/specials. Noted that when the discount plan is supplemental to your primary insurance plan, the coverage described above will come into effect once your insurance’s limits due to frequency occur. No refunds are given to patients, or rolled-over if patient chooses not to use their BITB dental plan. This plan does not apply to specialist referrals, and other fees may apply outside of Dental World Miami’s office.
